How does this work?
Register with our Office
Call our office at (435) 523-3375 and register with our office. You'll need to send us a photo of your insurance information (if you'd like us to bill your insurance) and your state-issued identification, passport, or driver's license.
Book Your Telehealth Visit
If we're billing your insurance, we'll first verify benefits, obtain any necessary pre-authorization and then call you with options for your appointment. If you plan to self-pay, we'll schedule you on the spot!
Speak to the Doctor
You'll first speak with the medical assistant who will enter your medical details into your record to prepare for your appointment. At the appointed time, you'll click a link we send you and the doctor will be online to visit with you.
What if I need xrays or surgery?
X-rays, MRIs or CT Scans
Depending on the nature of your concern, you may first visit with our Physician Assistant who will determine if any imaging studies are necessary. If' you've had recent x-rays, MRI or CT Scans, please request that they be sent to us in advance of your visit with the doctor.
What if I Need Surgery?
When you visit with the doctor, and review your imaging together, your doctor may recommend surgery. If that's the case, we'll work out all those details, (e.g., lab tests, authorizations, admission arrangements, etc.) at the conclusion of your call or shortly thereafter.
What if I Live Far Away?
Our staff will help you with all your travel arrangements, including transportation, accommodations, and ground transportation if you fly, and driving directions if you drive by car to our location. If you don't live near a large airport, we may arrange travel through AngelFlights West, a nonprofit, volunteer-driven organization that arranges free, non-emergency air travel for children and adults with medical needs.

X-rays, MRI or CT Scans
If you don't have health insurance, we can guide you to imaging centers that offer discounts for patients who pay in full at the time of service. We can also guide you to medical lenders who will grant you credit on very easy terms.
Predictable Surgery Pricing
Ask about surgery facilities that offer transparent, bundled price surgery that include facility fee, implants / hardware, surgeon's fee, anesthetist's fee, take home medications and even an overnight stay, if necessary.
